                                      COPAKE TREE TRIMMING FATALITY
                                                                       November 7, 2007


Copake, NY           Columbia County Sheriff David W. Harrison, Jr. reports that his office is investigating an
incident that resulted in the death of 21-year-old Jaime Perez-Carvajal of Massapequa, NY.   Tuesday, November
6th at 12:06pm, Columbia County 911 assigned the Copake Police Department to state route 22 and county Rte.
7A in the Town of Copake for report of a worker injured in a bucket truck.  Perez-Carvajal had been cutting trees
from within the raised bucket along route 22. A witness at the scene stated the bucket and boom rapidly came
down crashing into a guardrail.  The victim, an employee of Cheap Charlie’s Tree Service of Holbrook, NY which
owned the vehicle, was working for Liberty Tree Service of West Islip, NY at the time of the incident, who was
working under contract for the New York State Department of Transportation.

Members of the Copake Community Rescue Squad provided emergency medical services at the scene and
transported Perez-Carvajal to the Fairview Hospital in Great Barrington, Mass where he was later pronounced
dead.  An autopsy is being performed this afternoon at the Berkshire Medical Center in Pittsfield, Mass.

At the request of the Copake Police Department, the Sheriff’s Office conducted the investigation.
The O.S.H.A. and the NYS Department of Transportation Vehicle Inspection was notified and responded to the
scene.  Members of the Copake Fire Department also assisted at the scene.
